Requesting Access to Epiccare Link

Sending your referral through our Electronic Health Record (EHR) EpicCare Link System can improve care coordination, communication and timely access to care.

In order to submit referrals, you need to complete and submit a Participant Access Agreement (PAA) to our Provider Relations Team at [email protected]. They will process your request and contact you with information necessary to submit your referral in the EHR Epic Link System.
